I am a licensed Dentist in Texas since 2006 and I use all these forms.  These documents are not EXHAUSTIVE so please read them, edit them and THEN APPLY as needed in your own practice. These are in Word Format so you can edit as you please. Some are in PDF but can be edited as well. When editing; look for the “Search and Replace Option” in Word under Home Tab, and then search and replace for “Dental Office or Dental Office Name” and then you will be able to replace with your own Office Name etc. If editing in ADOBE, you will have to use and download ADOBE PRO DC—which is free for the first month. Some of these forms especially EMPLOYMENT HANDBOOK AND ALL THE CONTRACTS are written in compliance to TEXAS laws. Texas is an AT WILL EMPLOYMENT STATE meaning either party (employee and employer) can alter the relationship with or without prior notice. Please go to your STATE’s Workforce Commission’s website for details for your own state to customize the handbook.
